In May 1941, in an effort to organize the World War II (WWII) civil defense effort, President Franklin Roosevelt created the Office of Civilian Defense (OCD), and appointed New York Mayor Fiorello LaGuardia as its chief. The Mayor chose popular First Lady Eleanor Roosevelt as his assistant. She was put in charge of Voluntary Participation, with four units under her: Veteran Activities, Women's Activities, Youth Activities, and Physical Fitness. This is a generic report form where the local air raid warden was required to report the following information in the event of an attack: city or town; damage; time and date of incident; warden's post number; location of incident; type of bomb; number of casualties; whether there was a fire; type of damage done to water, gas, sewer, electric or telephone services; streets blocked, location of unexploded bombs; and services that were on the scene or on their way to the scene.
